I, the undersigned parent/guardian of the minor child named above, acknowledge that participation in basketball training involves inherent risks of injury. I voluntarily assume all such risks and release Jumpstart Hoops and their staff from any and all liability for injuries sustained during program sessions. I certify that the child is physically fit to participate in high-intensity athletic training and authorize emergency medical treatment if necessary. I consent to receive program-related emails including confirmations and class reminders.
